Zero clue what you put down as the dob. Tried www.yellowpages.co.nz to try to find them?

I believe its your choice who you add as far as family members. However if you don't name someone who then wants to come out off the back of you questions may be asked. I just put, my dad, sister and brother.
Hope this makes sense.

Re: Family members!!!
I just put brothers and sisters, hubby comes from a large family so it took a while to get all birth dates together including his half brothers and sisters in Oz.
I def didn't bother with cousins and stuff

In th eguide to filling in the form it tells you that brothers, sisters, parents and children are "family"
basically i think it only those who they feel may be relevent to your medical history - I think.
