NHS for the spouse visa applicant?
Once my wife has paid the NHS fee and is granted her spouse visa, how does she go about receiving care in the UK? Is this different from me as the citizen?

She finds a local General Practitioner, just like you, and signs on. That's it.

I'm British, hubby NZ, he just registered with a gp and gets free healthcare like me. Prescriptions cost a lot but you can pay a subscription fee and get medication posted free. We downloaded 'Echo' app on phone and it all arrives in the mail on a regular basis. Very handy.
