The easiest thing for you to do is call your local APHIS office and they will send you the DEFRA forms and clear instructions on the timing etc and how to fill out the forms. They can also tell you if your vet is accredited. The when you book your flight the airline will send you the customs form and fitness to fly certificate along with any other paperwork they will need. I'm on my phone right now so can't really tell you the full process. If you want to PM me I will try to help once I can get to my computer.

Many vets don't know how to fill in the necessary DEFRA, etc. forms so you definitely need to keep an eye on this. It got to the point where my vet asked me to fill in the forms and she would sign them. This was back in the days of the pet passport process.

Our vet in Thousand Oaks did not know how to fill the forms in, but I had seen examples on the USDA site. The health certificate can only be filled out by a USDA vet and endorsed by the USDA federal vet, which for us was in Los Angeles and they did not except faxes or photo copies had to be the originals. There was a women their who had travelled seven hours by grey hound bus from Vegas to get hers endorsed only to find out they did not except cash only credit/debit cards. All her paper work was in order, the only thing was they would not except the cash, so we paid her $38 by credit card and she gave me the cash, I could not see her travel seven hours and back again just to get a credit card, apparently she had travelled down the week before only to find they were closed. They are open open from 7.30 am to 11am. The USDA endorsed the health certificate and the annex II, the c5custom form was needed BA Cargo we faxed it to James cargo before so they could give us the waybill number. BA sent us one too with our confirmation number.

I completed most of the form for the vet and then she pretty much just completed the rabies part and signed it.

Movers have come and gone! They started around 8:30 and were done by 2:30 so not bad timing at all. Very weird feeling to be in a completely house, some sadness as it was the first place that I owned and it was my own little hide away. Anyway its all done now and we will spend the next few days at a friends house until we fly. Not sure what I am feeling right now other than tired! Oh well, chinese food for dinner and a nice glass of wine heading my way!
