Is the child being born in the US? The child is a US citizen at birth. (except for
some diplomatic exceptions)
Is the child being born abroad with one US citizen parent. (except for some special
circumstance of a USC parent that never resided in the US) The child is a US citizen
at birth. You file the form for citizens born abroad. If the child is out of wedlock,
then further proof may be required.
What do you mean "what happens"?

And.. My favorite:
If the child is present (meeting the INA defintion found in 101(b)(1) in the US as a
legal permanent resident, and is adopted by a US citzen, that child is a US citizen.

That would be called immigraculate conception. Three Eastern Regional directors,
one each from FBI, CIA and INS will come to your home bearing various gifts and NO
additional forms for you to fill. All this is true only if the baby is born in the
USA, eventhough the manger may be "made in china" - as all mangers seems to be
these days.
