Use of old forms for AOS medical

I had my AOS medical a couple of weeks ago in Fairfax, VA. I took the
current forms (obtained from the USCIS website) with me for the doctor
to use. However, they said they didn't need them as they already had
the "INS" forms. I insisted that they use my forms, but they refused
and the situation was quickly developing into a shouting match. I
explained the importance of using the correct forms but they weren't
having any of it, and pretty much told me if I didn't like their forms,
I could leave and get the medical done elsewhere.
OK, so I backed-down and let them do the medical and returned a week
later to get the sealed envelope. They gave me a photocopy of what
they put in the envelope, and wouldn't you know it, the medical form
looks like a copy of a copy, of a copy, of a copy, and worst of all, it
has 'INS' all over it. I have know idea how old this form is, the copy
is so bad that the version info at the bottom of the form is not
readable. I questioned them about it and they said "...you'll be
fine..." and "...we use that form all the time and never have a
problem...".
Am I worrying about nothing? Having waited almost two years to get the
AOS interview, I don't want to be turned away on the day 'cause the
doctor used out-dated forms.

Hi there,
If you go to www.uscis.gov and look up the I-693 form there (I think that's the medical form), it should say which versions are still acceptable. USCIS sometimes has several versions of the same form floating out there, and all are acceptable. The form date should be in the lower right hand corner.

I checked out the download page for the I-693 and it does say that
previous versions are able to be used through 9/30/05, so I'll be OK
for our interview on Friday. I got the doctor's office to check their
most legible copy of the form and the revision date is 09/01/87 !!!!
Why do these folks use such old forms, have they not heard of the
internet? I guess it's not their job to stay up to date on these
things, but if they are going to insist on using 'their' forms, I think
they have a responsibility to remain current.
