Urgent Help - Original EAD and AP are stolen with the passport

I need some urgent help.

I came to the US on a K1 and applied for a AOS in San Fran. I was travelling to
Paris, France over New Years. I had my passport with the AP and my EAD card in my
handbag and my handbag was stolen. I went to the US embassy and they were extremely
helpful and issued me a travel document since I had photocopies of the AP and the EAD
card. Now that I am back in the US what all am I supposed to do.

1)To re-apply for the EAD card . Is it I-695 or I-765 ? I have seen conflicting
articles and am confused.
2) Do I have to inform the INS about the original AP being stolen ? The immigration
officer at SFO stamped my PP with the AP stamp and a validity marked "indef"
3) Do I have to inform the INS about the change of my PP no. since a new PP has
been issued ?

Any help will be appreciated. Thanks. RCS
