Starting AOS app for K-3

Greetings again,
My husband finally activated his K-3 visa on Aug 8. Poor border guards had never
seen one before, but at least they knew us from us crossing every single week for
almost a year! All in all it went fairly smoothly, except we didn't realize they
needed $6 cash for an inspection fee. Neither of us had enough money on us, but
thankfully there was an ATM at the duty free store down the street. So bam, my
hsuband has his A#, his I-94 card stamped with his multiple entry stamp, and is all
happily legal now!
We sent off my husband's EAD application to Chicago the next day, and are now getting
ready to start his AOS application. Most of it seems pretty straightforward, but I
did have a few questions which hopefully someone can answer.
First off, on Page 1 of the I-485, which box do I check for the reason for applying
for permanent resident? Our I-130 has not yet been approved.
Second, I have read that my husband shouldn't need a second medical exam. However,
he will need a vaccination supplement. Where does he get this and is there a
specific form for it?
I might think of other questions later on. Thanks alot to the people of this group
for all their help so far.
