Where do get a Spouse Visa application form? Hope I can download one off the web! Is
this the same as a petition for alien relative?

You need both a petition and visa application forms. The petition is the I-130 - petition for alien relative, and this must be accompanied by the G-325A, biographic data sheet. The visa application form is the OF-230 part i.

You can download them from the INS website.

I don't think the OF form is on the INS website, it isn't an INS form. And he really
doesn't need it until the interview date. It is generally sent by the consular staff
at the Embassy where the interview is.

He did ask for the *visa application form*, Michael. And the consulate won't necessarily know to invite him for interview unless he applies for the visa.

Indeed, OF-230(i) or now known as DS-230(i) must be sent back to the foreign consulate before an interview date is scheduled (at least in London anyway).

London allows one to send the I-130 to the INS and also send an OF-230(i)[visa branch] before INS approval so a provisional file can be opened. If not; OF-230(i) comes with packet 3 along with the I-864 and should be sent back then.
