spouse name question
In section 4 of the form (spouse name), should I write down
1- her firstname only
2- her maiden name and firstname
3- her spouse name (my lastname) and firstname

Sorry I didn't mention that I was talking about the DV2004 form...

Personally, I've never filled out a DV form, but I assume you would need to
enter your spouse's full name that she uses currently (her married name).

enter your spouse's full name that she uses currently (her married name).
