San Jose AOS interview on 5th March , help needed.

hi everybody, me and my wife have the famous adjustment of status
interview at San Jose office on 5th of March, i came here on K1 VISA
and so did have my medical exam. in my home country but the doctor who
conducted that exam didn't give me any vaccination shots saying that i
dont need them but he didn't give me any form, or any documentation
for that.
I wonder should i get my vaccination done here or should i go to
interview and tell them that i didn't require any vaccinations as told
to me by the doctor?
how does this thing affect my AOS i-551 stamp(if at all i get it that
day)?

Yes, you do need that form filled out. It should've been done in your
country and brought back with you. I looked for the form "Supplemental
Form to I-693" on the Internet and came up with this on the INS
website
"Note that the vaccination supplement accompanying Form I-693 is not
included on this internet posting. The Centers for Disease Control and
Prevention (CDC) have sent the vaccination supplement directly to the
civil surgeons for them to photocopy, fill out on behalf of each
applicant, and attach to Form I-693 for submission to INS."
So it sounds like an approved INS Doctor in your area should have
the forms in stock. You should also be able to get a form at your
local INS office.
