In case anyone is interested in how it turned out:
August 26 - interview at embassy, we were given 221g refusal letter
September 1 - sent joint sponsor's i-184 by royal mail special delivery to embassy
September 2 - documents delivered to embassy according to royal mail
September 6 - automated email asking us to send additional documents... this freaked us out completely, but turns out to have just been a duplicate of what had been sent before, still not sure why they sent it to us twice
September 7 - status changed to ISSUED on CEAC website
September 9 - email from courier
September 12 (today) - visa and passport received!!!
All quite smooth, quick and easy after all despite stress levels being very very high!
Everything all set for our departure on Friday
Thank you all for your help and reassuring us, especially Rene.

It is an envelope from the US Consulate that contains every piece of paper that has been submitted for your visa. It gets handed to the officer at the POE and he/she opens it, makes notes, stamps your passport and retains the envelope and passes it on to the USCIS for filing. DO NOT LOSE IT.

I was so excited when mine turned up... which quickly changed when I saw it is literally just a white envelope with nothing on it (I was expecting it to have the dept of state seal on it or something). I was worried that either Heathrow or Virgin security would do something dumb during bag checks - so I made a point of telling them it was visa information at the pre-board security checks. They didn't care (probably internally eye rolling) - but I was paranoid the seal would get damaged as they poke around your carry-ons.
