Default Question about marriage

Hi, Everyone. I have some questions. First of all, I have a green card.
I got married and changed my last name on my SSC. And I went to driver's
license to change my last name on Driver's license. As I was told from
court, I was doing right steps to chnage my last name. Well, at
driver's license place, they told me that I have to change my last name
on my GC first then Driver's license unless I was U.S citizen.

I called the 1800 number they told me to file the application (I-90) to
change my name.

This is my question.

1) at family name on application, Do I put my husband's name? or my
maiden name as it's shown on my GC?

2) On the naxt page, it says " if you entered USA with an immigrant
visa, complete following." I didn't come with immigrant visa. Should
I leave blank?

I don't want to make a mistake because I know how they deal with
mistaken application.....DELAY my application.

SO, Please tell me what to do. Thanks

1. Put your married (huband's) name there.

2. How and where did you enter?
