Default question about B2/I94

A friend of mine got B2 visa for 6 months but upon arrival, the inspector
wrote 'NO COS EOS' on I94. He went here many times in the past, and each
time he legally entered the country and departed on time without any status
violation. But this time, the inspector wrote down this, was saying no
change of status, no extend of stay. Does it mean he can not file for
extension of status? It will be automatically be denied even if he file for
B2 extension in 6 months. Any insight or explanation will be appreciated.

The inspector didn't trust him.

It means exactly what it says.

This annotation is a recommendation, not an absolute prohibition on
EOS/COS. You can still apply, but there is a good chance it will be denied.
