We are about to file for an ajustment of status(I-485). Im just confused
co'z I already had my medical exam in the Philippines before going here
in US since Im holding a K3 (spouse)visa. Do I need to submit a I-693 or
can I just submit my vaccination documentation worksheet from the
Philippines?

I hope someone would help me on this plsssssssss.

You need to get a "vaccination supplement" done by a USCIS approved doctor in the USA in your area. You do not need a whole new medical. Bring your vaccination card from the PI with you to the doctor. He will look at it, he will fill out a new form, put it in a sealed envelope, and you will send that sealed envelope with your AOS package.
