Packet 4 ? URGENT
Hi,
I have my interview at the London Embassy soon and do not appear to have the form 9003 'Additional Questions to be completed by all applicants for permanent residency in the US'
My questions are:
What questions are asked on this form - do I need to provide documented evidence with this form?
Is it a long form?
Can I download it from anywhere?
Am I right in thinking the form is something to do with the IRS?
Any help will be gratefully received

As far as I am aware, this form is only required at the adjustment of status stage,
once you are in the US. But I could be wrong. Here's a link:

I've just had my interview in London today.
I asked this question about the 9003 some time back and was
told it is not required.
I know it is mentioned in Packet 4 but ignore that, it is old
information and certainly no mention was made of it today at
my interview.

I have an interview in august and I never got that form and there is no mention off
it, if you are worried give them a phone and ask them, they are very nice but at
£1.50 a minute they would have to be
