Our notice of action expires soon and have not gotten an interview date as the London
embassy "says" they did not get my fiancess paperwork Does anyone know the name of
the form I need to fill out to get my date extended or even if such a form exsists?

I think that what you need is a letter of intent and not a form...read the K-1 link

sounds like you need to write a notarized "letter of intent" to the consulate.

What paper work have they not gotten, the forms he needs to fill out and return to them (packet three) or the ones from the service center? Is it possible for you to fax them a copy of the notice of action? To get things started if you haven't yet. you may want to try to contact the service center to see if they have sent the stuff off to London or what is going on.
