We had our medical and interview in London yesterday for spouse visa.
Apart from the waiting around it was very painless.
As regards the ADIT pictures, I took them myself with a digital camera
and sized them up in Photoshop. No problem with them at all.
And no problem using an Advanced Discloure from the CRB instead of a
Police certificate.
We hit the doctors at 7:59 and were out of the Embassy all done at
11:20.

Very straightforward. Although we live in the UK my wife didn't come
to London with me. We had the I-864 forms notarised when we were in
Texas in January.
The guy asked me if my wife lived in the UK. (He knew the answer to
that already)
How long we had been married. (He knew the answer to that one too)
Where we were going to live in the US. (Guess what, that was on the
forms he had in front of him too so he knew the answer)
He asked me if both of us were intending to work in the US. I told
him "only if I have to"
And then he asked my 11 year old which was better. Disneyland or
DisneyWorld.. Was she looking forward to living in the States.
and that was it. 5 minutes, tops.
I had all the paperwork in the correct order in a binder in plastic
pockets. They seem to appreciate that. I couldn't believe that there
were people in the doctor's watingi room filling out the I-864 !!!
As I said, the whole thing was easy. Just hot in the Embassy. Take
some bottled water and a book to read and you will be fine.

The timeline would not be useful as I applied at the end of last year
and didn't want in interview until May, so they held of scheduling one
until then. They will only give you an interview a month before you
intend to travel.
so my timeline of seven months from application to visa issue is not
representative of the true situation.

My wife and I did DCF in London in 2001. The consular staff, in my
experience, are not out to trip anyone up. They seem to try to help
people obtain visas, not deny them. Of course, not every visa is
approved, I am simply commenting on the general attitude I witnessed.
Ours was approved and they "waived" the employer letter requirement even
when it was my fault that I did not have one on the day
Good luck
