I'm due to have the interview in a few weeks and I'm starting to get a
little worried about the chest x-ray.
I know this sounds completely crazy but......
Although I've recently given up (6 months and counting), I was quite a
heavy smoker for about 20 years or so and I'm worried that any damage
my lungs have suffered over the years will appear on the x-ray and
therefore preclude me from the visa.
Has anyone else had the same thoughts or am I just worrying about
nothing?

. Anyway, what they are looking for is TB, STDs and any other chronic health problem that would make you a "danger" to the public at large, aka Typhoid Mary syndrome.

The X-ray is for TB. I don't remember hearing of anyone refused for
potential problems due to smoking before but perhaps a google search will
see if there are other cases.

I really have no clue what they look for when they perform x-rays but
one thing I know is that most damage done to your lungs by smoking is
reversible. Understand: If you really gave up smoking 6 months ago,
your lungs shouldn't look that bad on those xrays :-)

I'll second this. As a smoker, I had no problems whatsoever. The doc asked me if I smoke, but didn't bat an eyelid when I said yes.

My husband at the time smoked three packs a day and didn't have a problem. Note that one poster from Canada (her names begins with an E) had the xrays done and they found a heart problem.
