If your daughter is your agent then she'll get a copy of everything before you. You can request the appointment letter to be emailed, but again they may only send it to her. The letter is 2 pages with 2 more pages of additional info. The rest of the paperwork for the appointment has to be downloaded from the Dept. of State Website. The give the web address in the letter. It is: http://travel.state.gov/visa/immigra...info_1335.html you'll find everything else you need including the info on the medical and how to make you appointment. The medical questionnaire is at the end of the medical info pdf file.
I'd also give them a few days after your case is complete before you ask for your appointment date as it can take a few days before that happens.
