I751 docs and form

Hi everyone.So I've alomost assembled my I751 docs
We are just going to use clips and not acco fasteners since Mr.F mentioned they are now scanning docs. Most of the docs all are from just after we got married till present.A few from before as well. I am including 4 per year of any statements or bills.
Here is what we have
Copy front and bank of husbands greencard
Copys of both of our NY State IDS
Health insurance letter verifying that I added my husband
Copies of health insurance cards
Health insurance letter varifying name change
Letter stating husband is authorised user on credit card
Copies of credit cards with same account number
lease with both our names (2 year renewal lease from 2009)
Statement that my husband is beneficiary on my IRA
Letter that I am beneficiary on my husbands 401K
A letter from my super verifying that my husband lives with me and when he moved in.
Letters from both our jobs listing each other as emergency contacts
Both our names on Con Ed, Phone and Cable bills
Tax transcripts for 2008 and 2009 married filed jointly.

announcement of our marriage in Scottish newspaper
pictures from the past few years
copies of announcements from pre wedding party in Scotland and wedding reception here.
E tickets from a few holidays over the past few years. Mainly after we were married.
Copies of cards and letters addressed to us both
Hotel receipts from England and Scotland
Copies of travel insurance from trip to Scotland recently.
Joint bank staements.
Check for $590.
We don't have affidavits . I could get them but I don't think I need them.
Planning to mail it on Monday!
Oh on part for where it asks for my family name do I put my married name or married with maiden name Smith (nee Brown) ?

Forgot to mention a cover letter listing what is incuded. Also should I put his A number on each page?

Don't bother with affidavit, it's weak sauce anyway.
You've included pretty much what I included.
With the utilities, I had alternated between each utility type each month, so one month was the cable, the next the oil, next the electric etc.
We did that because some were in my name, some the missus and a couple in both and we only included the previous two years worth, but we had moved states, so used repeats of work/insurance etc for the first year.
If you've got a car, it's worth a copy of the registration and insurance.

If there's a place for "other names used", put any other names you've been known by.

It's not my greencard its my husbands. On part four of the form it asks for spouses family name
