Default I have a few questions about the I-751?

On part 4, what is the family name? Is it the name before I married my husband when I was married to my ex husband or is it my maiden name before I ever got married or is it my name now?

On part 5 it ask about your children. Does it mean the children born into the marriage or could it mean the children I had before I married my husband? I and my children were all born in the United States.

Also, I live in Texas and I was told not to mail my I-751 to the TSC because they are forwarding all of the petitions to Vermont and it only slows things down. Do you think it would hurt if I mail it to Vermont instead of Texas?

Who do I make my money order out to, the USCIS or Department of Homeland Security? Thanks for taking the time to read this. I am new to the forum and I had a few questions.

I used my current family name. When I got married, I took my husband's last name. That's what I put here.

We don't have any children, but I think if I were you, I'd list them all.

Well then... I'm totally confused. If you were born in the US - you're a US citizen... so why are you filling out an I-751?

Thanks... I understand the concept! However, the OP refers to "my I-751".
