Question 11: Have you ever applied for employment authorization from USCIS?

Are they including applying for an H1-B? As I am here on an H1B. They also ask for which USCIS office??

The employment auth of your H-1B is built into the status/visa. (yes that's poorly worded)
You applied for your visa through DOS, not USCIS.
