Default i-765 Application for Employment Authorization Question

I was on a B-2 visa and overstayed. I got married to a US Citizen and
am applying to adjust status with an I-485 package.

I was a but confused by Question 16. of the i-756 form. There are
three ( ) ( ) ( )

fields.

Do I simply notate the following (which I got from page 4 of the i-765
package:

( C 9 ) ( ) ( )

Do I need any other codes in the other two blanks if I file this
application with my I-485 package?

Or should it be:

( C ) ( 9 ) ( )

Thanks,

-David
