I-751 Affidavit info please

Hi,
Do the affidavits for the I-751 need to be in any specific format (besides the
requirements listed in the I-751's instructions, like the person's date and place of
birth, etc..)?
Do they need to be notarized or anything?
Any info appreciated.

Hi Todd,
We just made sure that the affidavits included all the info mentioned in the
instructions and had them notarized. It was just like a "To whom It May Concern"
kind of letter format.

An immigration lawyer notes:
The law does NOT require notarized affidavits. The legal equivalent of an affidavit
will have the following format:
" I, John Smith, declare:
[factual statements].
I declare under penalty of perjury under the laws of the United States of America
that the foregoing is true and correct. Executed on [date] at [place].
[signature]"
As a habit from my litigation practice, I like to include the following allegation
"All the following is true of my own personal knowledge except for those matters
identified as being of my belief. I believew that all matters identified as being of
my belief are true."
Note that the INS forms themselves include the magic language about declarations.
Before they put in that language, many of the forms had to be notarized.
