I-485 Vaccination Supplement BLANK!! PLS HELP

Default I-485 Vaccination Supplement BLANK!! PLS HELP

My wife entered the U.S on a K1 visa.
We married three weeks ago.
I am gathering the documentation, forms, etc. for her AOS.
One document needed is the vaccination supplement from her
medical exam. We have this document.
The doctor signed and dated the document.
Her name, case number, etc. is filled in, but the vaccination history
section is BLANK.

Is this going to be a problem when we send in her I-485 application?
Or, is it common to leave the vaccination history blank?
We are not the only ones as we saw others (going through the process in
Ciudad Juarez) with blank vaccination histories too.
If it will be a problem, what can we do?
Should I fill in the vaccination history from her shot records?
Should we just send copies of her shot records with the I-485?

Is there anyone here who has gone through the same problem?

Advice is greatly appreciated..

I remember having a similar issue when I was preparing the I-485
package for my wife, who came to the U.S. on K1. The instructions
stated that for the finacee visa, a complete medical exam was not
necessary since this was done prior to her coming here, but that she
would need a *vaccination supplement.* It wasn't clear to us if this
was some kind of official form, or if it just meant that we needed to
provide some evidence of vaccination. We figured out from online
research that it WAS an official form, and although we did have
vaccination records, we did not have an official vaccination
supplement. We called the BCIS automated phone system that gives you
the lists of certified doctors near you. We made an appointment with
that doctor's office and told them that we needed only the vaccination
supplement. This seemed like a new type of request to them (they are
used to doing the whole shebang of a BCIS medical exam), but they
understood and even had the blank vaccination supplement forms there.
The doctor visit cost like $75 or something. What we got from that
doctor visit was an official vaccination supplement, complete, signed,
and sealed in an envelope for the BCIS. It being sealed to show we
hadn't tampered with it seemed important. (And the doctor gave us a
copy that we were able to look at.) This seemed like the safest bet
to us. I hope I am correctly interpreting your situation to be
similar to ours.
