Hi.... I'm sorry for flooding this NG with all my questions... but I'm so helpless...
My husband was born outside of USA. He has FS-240 and US passport, but he can't find
a certificate of citizenship. In question 13, they ask you if you have "obtained a
certificate of citizenship in your own name." What is this certificate of
citizenship? If you have FS-240, will your answer be "no?" Or does he suppose to have
the certificate of citizenship along with FS-240, but not finding one?

http://www.passportexpress.com/abroad.asp IF he was issued a passport, then he surely
has a birth certificate that was also issued, I *think*. Those URL's have the
procedure for obtaining the birth certificate.

The FS-240 is a consular report of birth abroad - basically his US birth certificate, indicating that he is a US citizen from birth.
A certificate of citizenship is issued to someone who acquired their US citizenship as a result of naturalisation. Therefore the certificate of citizenship and FS-240 are mutually exclusive - you can't have both.
The blurb about having a certificate of citizenship issued in his own name is referring to someone who became a citizen when their parents were naturalised - derivative citizenship. This does not concern you.
