Default Go medical summary

Hi all, I've just picked up my medical summary and wanted to know by your experiences I have the right document.
Its 2 A4 pieces of paper stating ,

Name and Address
Problems active
significant past
medication
allergies
health status and immunization..

thats all whats on it really, clippets of information with dates. Is this all I need ??? Also it's dated 06 Nov 2018 , is tthere a expiry date for the summary due to medical exam in the coming months.....regards.

When is your appointment for your medical at Knightsbridge? Since the medical is good for six months from the date of the exam, assume the summary will be as well.

No appointment as of yet, we are completing all paper work by the end of December (DS260) ( AOS) , due to an already planned visit and already paid for trip , couldn't really submit it now due to this. The plan was to complete paper work and hopefully get the interview maybe late January or February !!! Hopefully...

My medical summary was dated June 7th, and was fine for use at my medical on October 4th. Knightsbridge did request further information from my GP relating to a specific health issue, so they ended up with a later-dated summary, but I was told that my original one would have been fine had they not needed more info. Based on that, I would assume that if your medical is likely to take place within the next four months, you should be fine. You could call Knightsbridge and ask if it would put your mind at ease, I'm sure they'd be able to tell you.

Thanks, that sounds like what I was hoping for, my only concern was it stated on my summary epilepsy, the last account of that was in 2002 over 16 yrs ago, so going back to the gp next week to add more regarding this, as I've never been on any medication, no test or follow ups., and I always believed it was stress... So I'm hoping he can amend it, or at least say I'm fine. Also checked on Google I'm sure the above will be fine anyways with application.
