G325A questions.. again.. sorry....

i'm sorry folks.. i know i've asked this question before but i've obviously
buggered up and forgotten to save the answer somewhere...
my daughters going to be included in the I-129f application.. do we have to
fill out a G325A for her as well?
also..
both my daughter and I reverted to my maiden name after I divorced 10 years
ago..
should we include in the I-129F copies of our birth certificates and
statutory declarations regarding the change of name... both of our passports
are in my maiden name now...
neither Tank (USC) or i can see anywhere on the I-129f the requirement for
my birth certificate or my daughters.... but perhaps if we send these doc's
it'll save an RFE...
i've also obtained a copy of the Order that was made at the time of my
divorce... no mention is made at all of my daughter... neither Custody or
Parental Responsibility were split between us.. there is NO mention of him
having contact or parental responsibility... is it still going to be
necessary to get his permission for her to leave the country...

Your daughter's name needs to be on the I-129f petition whether or not she will apply for a visa. But she is not involved until, and if, she does apply.
I haven't read of it happening recently, but there have been reports of RFE's for the beneficiary's birth certificate in the past. So even though not required by the instructions, it wouldn't hurt to include a copy of your birth certificate.
The I-129f requires you to list all names you've used. It doesn't ask you to explain why you used those names.

As someone mentioned, the children are on the I-129F whether they are
coming or not.
I believe 325A is for the ones that are 14 or over (that are
accompanying you)
