I am in the US on K-1. I want to change my name after marriage.

For All Other Names Used, should I put my Maiden Name.

When it says in the form Applicant's Last Address Outside the United States of More than one Year - Should I put my address before I came to US or should I leave it blank.

This form is submitted in connnection with Application for - I should tick Status as Permanent Resident. Please confirm.

Yes to all 3 questions - put your maiden name under "other names used", your address in your homeland and select "App. for perm. resident"

Put your last address of at least a year outside of the US
