G-325A Question - Employers' Addresses?

Ok, one more question before I pop this stuff in the mail.
Form G-325A, Biographic Information. It asks for a 5-year employment
history, including the name and address of each employer.
1) Do they want the local address of the office in which I worked, or do
they want the corporate address? The corporate headquarters (the address on
my old pay stubs) are all over the country, so I don't want to raise a flag
by putting in the corporate addresses when they want the actual office where
I worked every day.
2) How strict are they on this information? What is it used for? I ask
because I've got 5 lines to fill, and have actually had 6 jobs in the last 5
years. One of the jobs was only for a month, and it was actually with a
company listed higher up on the list (when I got a different job with them
later). So technically, my last 5 years of companies is listed, but not
that one-month position. Is it worth adding a continuation sheet, or should
I just leave it?

Zed - My husband had this question, too. He is a sales rep, who worked
primarily from his home office, but we listed the local address for his
national company, which was not the same as the one on his paystubs.
This was for both the consulate and AOS parts, and we had no problems.
We also ran out of space to put the addresses (we listed the full
mailing address for each and they wouldn't fit), so we just put the name
of the employer on each line, with "see attached" in the address column,
and then filled in the rest of the info. On the attachment, we included
the complete mailing address. I presume you could simply leave the last
line blank, but say "Please see attached" for that, too, and simply list
all the info there.
As for whether you need to include that one month job, I'm not sure -
it's really your call. If it were me, I'd list it because they are
asking for all your employment for the past five years. I don't think
it's a huge deal, though. Good luck with getting the paperwork done and
