In the first box of the G-325A form (family name), upper left-hand side, does my new
wife use her married name or her maiden name? Also, is it necessary to submit this
form again, since we submitted it before with the I-129F?........Thank you :)

I used my new married name. There is another box where she can put all her other
names used (my maiden name was there). :)
