Last night I started the AOS paperwork for my wife. We married on Jan.
17 and her K-1 visa expires on Feb 15th. I have a few questions now and
know I will have more as we progress through this mess. I would
appreciate any help and pointers along the way!

First question:
Do I use her married name or maiden name on the forms for last name? I
saw where one form wanted her name exactly as it was on the I-94, but the
others are not very self-explanatory.

Is there a website that shows examples of all the forms (completed) for
us to use as examples?

I read on the I-485 instructions we could submit a vaccination
supplement in place of a new medical exam. IF it was less than one year
since her K-1 exam. Is that one year at the time of interview or when we
submit the AOS package??


There are so many questions when I am working on the forms and I can't
think of them now! Some are stupid, but I want to make sure I have the
paperwork correct. Please forgive my foolish questions!

We used my married name on the AOS papers.
I don't know answers to the other questions, so I'm bumping your post up for others to read.
