Hello,
I am about to send in the I129F for my fiance who is in Canada.
However, her last name is the same as her father's first name (not his
last)...should we explain this in a cover letter or will it be ok?
Thanks,
Osman

Don't worry about it. In dealing with the INS it is best not to tell them more than they need to know - anything more is just likely to cause concerns where there should be none.

I agree. My wife's last name was identical to her father's first name
not her father's last name. That's just her custom. We simply filled
the applications out with true/correct info, nothing special, and it
so far has never posed a problem.

There is nothing that requires that there be an identical family surname. In some societies, what you describe is the way daughter's are named -- Iceland is an example that comes readily to mind.
Its not a problem.

I wouldn't worry about her last name being the same as her father's first.
That is EXACTLY what happened to me. My fathers last name was not my last name. It was a strange deal, but anyway, it didn't matter to BCIS.
Best of luck

It's not a problem.
My husband's name didn't match his father's name and my stepson's
last name was taken from his mother not his father.
Don't worry!
