Hi there,
I've been a permanent resident since june 2003 and understand that I have to apply for the restrictions to be removed within 3 months of the expiration date.
Which form do I need?
Also in my passport i have a stamp I-55 wich is valid until 6/16/04. Do I have to re-apply for this or is it void now that I have my social security number?
Finally, my wife and I are going for a visit to the U.K. in May, do i have to get any special forms for this trip or can I just show my Permanent Resident Card on the way back into the U.S. ??
Any help will be greatly appreciated.

The District Adjudicating Officer who interviewed probably told you that you would have to file for removal of conditions and that you can do so by filing form I-751 within 90 days of the expiration of the two year green card. Since you have been a CPR since June 2003 that means you would apply sometime in March 2005. Can't be more specific as I don't know the actual date of your approval.
The I551 stamp in your passport means nothing now as you have the physical green card as per your post. The I-551 stamp is only the paper equivalent of the card and is used to show your status until the card is received. The social security card is not USCIS issued and is not a factor at all in your questions.
As a CPR you don't need advance parole, your green card is all you need along with a valid passport. You would only need to file form I-131 for a re-entry permit if you planned on being out of the US for a year or more.
