Just filling in DS-230 part 1 and need a bit of help.
At part 35 I have to state all previous visits to the USA. We have been several times over the last few years but I cannot remember all the specific dates - will it be ok just to list the visits that I have dates for?

Also, photos are required for all of us (family of 4) and I have had these taken to the specification (2 x 2 inch) but do I need to note the back of them with our names or anything?

No. You are supposed to list ALL visits. Try to remember the dates to the best of your ability. Try looking back at old passports or travel logs, diaries, something, to help jog your memory.

Only if the instructions tell you to.
