Default DS-156 for renewal appointment

I'm filling in my DS-156 and Section 29 has me a little stumped.

This is for my L1-B renewal and I've lived in the US for nearly 3 years now.

Section 29:
Have you ever been to the US?:Yes
When (most recent)?: I've used dates for the last time I entered the US (Nov/25/2007)
For How Long?: I've entered the date that I am leaving the US to visit the UK Aug/01/2008

Then the confusing part, it asks about previous visits. Do I put every time I've re-entered the US since the issuance of the visa??

This is a multi-purpose form and some parts will not apply to you. Just leave them blank - they can be amended at the interview if need be.
