I was looking at the Guatemalan embassy web site and they said that
Gabby will need to bring
"2 completed copies of the Non-Immigrant Visa Application Form DS-156.
This form is provided by the Consular Section but can also be obtained
from any branch of Banco Uno (where the fee is paid) or outside the
embassy Consular Section."
This is for her interview. Does anyone know if this form is
particular to Guatemala, or is available online somewhere?
Thanks!

This form will be sent to her in the Packet 3 from the consulate she has to
deal with. The packet will specify what needs to be returned and what she needs
to bring with her to the interview. All the consulates use this form.

Aaron,
My husband lives in Venezuela. Since the mail system there is not very reliable we perferred that he go personally and pick up the forms at the consulate. He took a copy of the 1st NOA (since he was not yet approved at the time), and picked up the forms in person, had absolutely no problems.
If distance is not a problem, I would truly recommend your fiance/wife to pick up the forms herself. It saves time!
