Cover letter for I-130/I-485 package

Cover letter for I-130/I-485 package
Hello,
In the case of filling I-130 and I-485 together should I include a single cover letter for both forms, or one for each form?
In the same package there will also be an I-765 and an I-131.
I got the model from the USCIS home page, and they inform to address the cover letter to the address of my service center... but I'm sending the package to the Chicago Lockbox. Which address should I use? Do I really have to put an address?

All of your applications will go in one envelope to the Chicago lockbox.
