Correction for an error in DHS record which affects my wife's I-130 visa

I am looking for help and advises on making progress in doing a
correction for an error in my wife's DHS record.
In 2005, we found there is an error that under my immigration record,
there is an extra record ( someone else's I-130 ) and the result of
this error caused my wife's immigration I-130 visa not able to be
granted due to a mark( fraud / misrepresentation ) found by her local
consulate office during my wife's recent immigration visa interview.
The consulate told me that we must contact DHS to have this information
cleared before they would grant my wife's visa.
I am facing this big issue about how to contact DHS to clear my wife's
record and what I can do in the mean time. It's so unfortunate that we
have to deal with such an accusation despite the fact that we know that
we did not do anything wrong.
