Anyway, I would do as you propose to do and send the DS-3025 that you have along with your I-485. That's what my ex-wife did, and it was all that was required of her. As I recall, she was given 4 vaccinations at her medical exam and all the rest were marked as medically inappropriate.

I think you are on to somthing here. IF the appropriate box on the DS3025 is noted/checked "All Vaccinations recieved and All vaccination requirement met" I THINK that's All that's nessesary. The needing of the Vacc Supplement is ONLY required if, for some reason, the petitioner has NOT had all the ness vaccines. I do remember Mr P and SC discussing this. They mentioned that the DS3025 they received did have this box noted/checked at the bottom indicating "All Vacc rec/ all Vacc rq met." In our case, the doctor did NOT note/check the appropriate box. That's why we went ahead and did the Vacc Supplement last Tues. You could be right, for some, the DS3025 may very well be all they need.

I received a DS-3025, however I didn't submit it with my I-485 (I was
told by a BCIS officer that a vaccination supplement was not needed at
that time). My DS-3025 is dated 10th April 2003 (not February, as I
thought in another thread), and my AOS interview is on 6th April 2004.
Do I need to obtain the I-693 supplement, or can I present the DS-3025
at the interview? (Processing centre is Newark, NJ).

My ex-wife did her AOS at Cherry Hill, a sub-office of Newark. She included her DS-3025 with her I-485, and was not asked for anything more at her interview.

Since you did not include your DS-3025 with your I-485, I would bring it with you to your interview.

As to whether or not they will want an I-693 from you, did you receive the checklist of items to bring with you to the interview? Cherry Hill gives such a checklist, identifying missing items they want brought to the interview.
