Default Conditional Green Card interview!

Hi folks,

My conditional GC is expiring January 10,2005.

I remember that they said I am suppossed to apply for a permanent GC
(in other words, to remove the conditions attached to my current GC)
90 days before my expiry date.

Could you please let me know what is the form I should fill out, what
documents should I carry and how does the process work?

I would really appreciate your comments.
