Default Change Address & Name on filing I-751

Hi Experts,

Its almost time for me to file I-751 or for the removal of my
conditional PR status. When i filed I-485/I-330 before i used my
maiden name but during the interview i requested if they can change it
to my husbands last name, they didnt. All my other id's like DL & SSN
has my husbands last name. I used it in signing for everything now.
So my question is can i put my new last name in the application
instead of my maiden name? Can you please tell me what to do?
Another thing is we moved to a new residence just a few days ago can i
used my new address in I-751 instead of filing for change of address
form?

I will appreciate all input.
thanks,
angel

I'm wondering about this too, please! As I filed AOS/EAD in my maiden name because they didn't change it in time for when I filed.

I think what I would do is use the maiden name in parentheses, like this. For example, let's say your maiden name was Smith and your married name is Johnson......

Susan (Smith) Johnson

I would also explain in the cover letter that you are now using your married name.

Thanks. Except in the form theres boxes for Last Name, Given Name,
Middle Initial? I guess explaining it in a letter will help
