Default can you use ALL the forms off the INS website? even the medical???????

I've posted this question before but didn't recieve a reply so I'll
try again in hopes.
Anyway, I have gotten all my paper work online at the INS website.
They have a form which you can print out for the medical. However
when I went for my medical I guess they were expecting the carbon copy
that they give you when you apply for it. Well I went online and
applied for it and gave them my address, it's almost been two weeks
now and nothing yet. So i'm wondering if the doctor can just use the
form that I got off the INS website if he made 4 copies of it?
My blood work still hasnt' come in yet, so I'm hoping that the paper
work might come by mail yet, but if it doesn't I was wondering if you
could use the downloaded version off the INS website.

Thanks

I would think that if they have a downloadable version on the INS website,
then you can use it.. it might be a little more of a hassle for the doctor
tho to have to fill it out 4 times, might not be pleased!

Actually I thought the INS doctors had their own forms and that you didn't have to bring any forms to the medical.
