Does anyone know if we can extend J1 visa and if so do we still have to leave the country to do it?? Can we also swap employees??

I hope someone can advise me?

Only your J-1 sponsor can extend J-1 status. J-1 status can be extended
only if you have not yet reached the maximum duration for the particular J-1
category that you are in.

If you are eligible for extension, it can be done without leaving the U.S.

J-1 transfers can be performed under some circumstances.

What is your category? J-1 trainee? J-1 research scholar? There are
several categories. Also, is your J-1 sponsor your employer, or is it an
independent J-1 sponsoring agency?.

I assume you have an J1 in the "trainee" Kategory?
If yes: You can extend your stay up to 18 months. You can only change employers, if you get a new DS 2019 for the new position issued.

If you don't plan to travel, you don't need a new visa. You just need to stay in status, which you are when you always have an updated DS 2019 without any gaps.
