AOS: New medical exam?

Hello everybody,
We're getting ready to file my AOS. Do any of you know whether or not I need
to have another medical exam (form I-693) or not? I'm receiving all kinds of
conflicting information on this both from websites AND the INS themselves.

I read somewhere that one is needed if it's been over a year since your last
one, if you came to the US on a K-1.
I do not know this 100% & would like to hear confirmation.
You do have to show proof of vaccinations. Whether your original vaccination
document will work or you need one from the US seems to vary.
I would like to hear the truth about this as well.

Perhaps you have come across this already but it is from INS' own site and says that you do not have to have another one if had one prior to admission and if you are applying for AOS within one year of arrival, though you would need to provide a vaccination supplement.
