Default affidavit for I-751 (2 person who know us)

One of the evidence of relationship suggested in I-751 form is an
affidavits sworn by at least to person who has a personal knowledge of
our relationship & marriage, is there a form i can download at USINS?
or anybody here who can give us a sample format?

Keep in mind this is only a "suggested" piece of evidence. From what I have
read here over the years, this is one of the weakest types of evidence to
submit with the I-751. We had our I-751 (no interview) approved without
using "affidavits."

Hi:

Don't waste your time on declarations [in lieu of affidavits] unless you have almost no other documentation.

Thanks for all the input ))) You mean i dont have to include that
affidavit if i have enough proof? I have pictures, birth certificate
of our son, mortgage on both our names, joint savings & checking
accounts, tax return filed jointly, insurance policy that shows im his
beneficiary & a lot more! I really dont feel like asking anybody
(even friends) to sign an affidavit
