The only problem I remember seeing from a poster was related to an
overstay of over 180 days, and then leaving and trying to return on AP.

Ineligibility for re-entry due to 3/10 year 212(a)(9) bar which was not picked up by people who issued the parole [their mistake, your problem].
Old criminal conviction which alien "forgot" about and improved database caught it.
USC got angry and withdrew the petition while hubby was abroad.
Prior order of deportation which was not picke up post filing fingerprint check.
