Default adjustment of status for spouse

My wife entered the US as a conditional permanent resident (for being
married to me, a US citizen) almost two years ago, so it is almost
time for us to apply for adjustment of status to lawful permanent
resident for her. I have some questions about this process.

First, do we just have to submit form I-751 with supporting
documentation? In particular, is it true that there is no need to
submit financial info relating to my ability to support her, or any
medical information regarding her health?

Second, in the I-751 instructions, it says we must submit affidavits
from at least two people who can support the validity of our
relationship. Do these forms have to conform to any special
guidelines? Can they just be in standard letter form with the
required information in them? Do they need to be notarized or
witnessed in some other way, or is simply having the signature of
person writing the letter sufficient?

How long after submission of this form can we expect to hear back from
USCIS. We will be submitting it to the Nebraska Service Center.

That is correct. You do not need an affidavit of support or a medical.

However, tax returns are one way of showing your viable marriage. Include tax returns for the years between adjustment and filing the I-751 if you filed joint.

The instructions does not say you MUST include these. It is an option. If you want to, then your writers have to follow the requirements as listed on the instructions. Their names, addresses, date of birth and where borne must be included. No they do not have to be notarized.

Your wife will receive a notice of action about a month after filing which will serve as paper proof of the extension of her green card status.
