My own was in a school test when I put that a mushroom was a type of fugitive rather than fungi....

GCSE math coursework. Damn auto replace on WordStar changed "secs" to " sex". On all 25 pages

Maths teacher not amused. Word processors were not so common then. The concept of auto replace even less.

Some years ago I was involved in groundwater monitoring for the Nuclear Industry. Unfortunately the word processor we used at the time insisted on converting the word 'borehole' to 'brothel'. Each report had to be extensively checked before sending to the regulator as no one wanted to report that they regularly sampled the local brothels.
