We asked DVLA for a written letter, it only took around 3 weeks. When we were at the JPG the officer saw my old supplementary sheet the UK dropped in I think 2015 and said you could have used that instead of the letter, so I said it is out of date no longer used but she was adamant that would not be a problem.

Licence conversion needs to be done at the main Wangsa Maju office. First step is to get an appointment to submit your documents which can only be done in person by visiting the office.
I downloaded and filled out the necessary forms but they are also available at the door. I brought all my documents on the first "pre submission" visit.
Documents required:
JPJ form L1
JPJ form Lampiran B-2
Original passport and a copy
Original MM2H offer letter and a copy
Copy of MM2H visa in passport
Original Driving licence and a copy
Either a (old no longer valid) counterpart UK driving licence original plus copy, or a Certificate of Entitlement.
I also provided a printout of the DVLA "View your driving licence" from https://www.gov.uk/view-driving-licence with a code for them to view your licence details online. (This was recognised and caused much smiling from the lady behind the counter)
Passport photo with white background (not mentioned in any document but seems to be required)
Look for door 1 downstairs. Don't join the huge queue. Go to the left side of door 1 and tell the door custodian (scary lady with big gun) that you want to convert foreign licence. She will tell you to form or join a queue outside the door on the left and will then call you forward when the relevant window is free.
All the documents are examined and discussed with other officers and eventually an appointment to come and actually submit the same documents is given. In my case for 3 weeks later.
Groundhog day with the queues again. Same scary lady told me where to stand and I was called forward to a window where docs were inspected and discussed and finally I was given a queue number!
Once the queue number was called I presented all my docs for the third time and they were inspected again, discussed within the office and finally accepted. I was told to wait in the room and eventually called again to be given the originals back and an application receipt.
Next a minimum of 30 days wait while checking the JPJ website until making another intrepid visit to Wangsa Maju to collect the Malaysian licence, hopefully!

Hallelujah! Applied in 14 October, approval appeared on the website on 3 January. P licence collected on 4 January.
Collection seems to require standing in the regular endless queue outside door 1, but being 'old' seemed to be an acceptable reason to cut the queue. Talk to the big shotgun toting officer just inside the door. Queue 4 to collect a number first, then normal queue process. Collection took me about an hour.

Just to complete my story; I applied at Wangsa Maju September 15th and had been checking the JPJ portal daily for quite some time and had figured I would need to take a trip down there to inevitably find that they had lost my application.
Lo and behold on 3rd of February, over 4 and a half months later, the JPJ portal updates the status to Approved. If you are worried for the poor JPJ officers end of year bonus, don't, their KPI will be met as they helpfully updated my application date to 3rd of February too.
The license cost RM140 to pick-up, a bit of standard queuing and waiting, I've found that Wangsa Maju office between the morning and lunch time rush, so around 10am, is OK for queuing.
The license was issued for 2 years and to reiterate what I've said in my previous post, I applied using a faxed version of the DVLA's Confirmation of Driving License Details (I don't have a paper counterpart) but this doesn't seem to always be a proven formula. It could be a reason for receiving my license 1 month later than heliaviator yet had applied 1 month earlier.

Thanks for the update. I'm still waiting from November 1st having submitted the original Confirmation of Driving License as demanded by the bod at the desk. I had the Licence Counterpart with me but bod was not interested in that (depends who you get). I didn't log in during the first week or two which means that the log-in is void and tak boleh be re-jigged, so I have to call every week to check (once in a blue moon they even answer the phone which is rather nice). Meanwhile Hermann my stop-gap driver thinks he has a job for life.

Collected my Provisional Licence today. Applied November 1st 2021 with all original documentation. 5 months 18 days from a department that has cepat in their mission statement. Beat that!
